Eligibility Requirements:
· Currently employed as a licensed full-time Nurse or Teacher with at least 3 years experience
· Currently enrolled in a college or university with classes scheduled
· Must be a citizen or permanent resident of the United States of America

1. Professional Development Plan
-Describe your professional development plan in detail
-Address how the proposed course will advance your workplace or professional goals
-Include course/seminar information and the organization offering the education/training
2. Enrollment verification, class schedule and itemized tuition statement from the University/Organization providing courses. Please note that a generalized cost of attendance outline from the University’s website will not suffice.
3. Signed letter of support from Principal/Supervisor
-One page maximum
-Must be on official school/medical facility stationery
-Confirms job description, status, and length of employment
-Indicates support for the proposal and its impact on the applicant’s development
Scholarships will be awarded 3 times per year - Spring, Summer, and Fall Semesters- and will be paid directly to the school/organization providing the education/training. Scholarships are granted for only one semester at a time but applicants can reapply for additional semesters. Applicants granted scholarships will be required to provide full social security numbers and grant permission to have their names published on our website.
Submission deadlines:
Summer Semester - May 1st
Fall Semester - August 1st
Spring Semester - December 15th
Unfortunately, we are unable to fund undergraduate degrees.
